A properly sized tap sounds like a good option; I've pried them out in the past with a small chisel, carefully. Use the shaft to tap the new one home, maybe a small shot of WD40 in the bore to ease it along.

Starter? I feel there's not a better option than my adaptor + a new Bosch SR0425N starter for the Syncro.
